Creatine benefits beyond muscle: brain, bone, and energy without the gym

A woman stretching and relaxing at home in her living room, with no gym equipment in sight

Short version: The best-studied creatine benefits beyond muscle are for the brain. A 2024 review of 16 trials in 492 adults found a small, real boost to memory. Early studies also point to less mental fatigue. Without exercise, creatine did not build muscle or bone in a 2-year trial.

  • Brain and memory: the strongest non-gym evidence, and it does not need a workout.
  • Energy and sleep loss: small, short studies. Promising, not proven.
  • Mood and blood sugar: studied only alongside medicine or exercise.
  • Bone and longevity: no bone benefit in women, and the life span data is in mice.

Last updated October 2026.

Most people still picture creatine as a tub of powder in a gym bag.

But your brain uses creatine too. It is part of how cells store quick energy, and that is why researchers started testing it for memory, tiredness, and aging.

So what does creatine do if you never lift a weight? Here is an honest scorecard, benefit by benefit, with the study behind each one.

What are the benefits of creatine beyond muscle?

Creatine helps your cells recycle ATP, the fuel they burn for fast work. Muscle holds most of your creatine. The brain holds a smaller but busy supply.

Here is how the non-muscle research stacks up today:

  • Memory: moderate evidence of a small benefit.
  • Mental fatigue: early evidence from small trials.
  • Thinking after a bad night: early evidence from one well-run lab study.
  • Mood: one trial, as an add-on to an antidepressant.
  • Blood sugar: one small trial, paired with exercise.
  • Bone: no benefit in the long trials in women.
  • Longevity: data in mice only.

The rest of this post walks through each one, in that order.

Does creatine work if you don't work out?

For muscle and bone, no. For the brain, maybe.

The longest no-exercise trial gave 200 women past menopause 3 g of creatine a day for 2 years. They did not follow a training plan. Creatine did not change their bone, their lean mass, or their strength. See Sales et al. (J Gerontol A, 2020).

The brain is a different story. Memory gains in the trials below showed up in people who were not training. The pill did the work, not the gym.

If muscle is your goal, creatine works best with strength training. Our guide to what creatine does for women covers those lifting trials.

Can creatine help your brain and memory?

Yes, a little. This is the best-supported benefit outside the gym.

A 2024 review pooled 16 randomized trials with 492 adults aged 21 to 76. Creatine improved memory by a small amount, and the reviewers rated that finding moderate certainty. The benefit looked bigger in women. Attention and speed gains were rated low certainty. See Xu et al. (Front Nutr, 2024).

Diet seems to matter. Meat and fish are the main food sources of creatine, so vegetarians tend to start lower.

Does creatine help with energy or mental fatigue?

Possibly, for the mental kind of tired. The studies are small.

In a Japanese trial, people took 8 g of creatine a day for 5 days. Then they did simple math problems over and over. They reported less mental fatigue than on placebo, and brain scans hinted that they used oxygen better. See Watanabe et al. (Neurosci Res, 2002).

A 2023 trial tested 4 g a day for 6 months in 12 people with fatigue after COVID-19. General fatigue dropped after 3 months, compared with where they started. See Slankamenac et al. (Food Sci Nutr, 2023).

Two cautions. Twelve people is a very small trial. And the lead researcher advises a creatine maker. If you feel tired all the time, see your doctor first. Low iron, thyroid changes, and poor sleep are common causes in women.

Does creatine help with sleep?

It does not help you fall asleep. It may help you think after you lose sleep.

A 2024 lab study kept people awake for about 21 hours. A single large dose of creatine, 0.35 g per kg of body weight, improved their thinking speed and test scores. Brain scans showed shifts in energy chemistry. See Gordji-Nejad et al. (Sci Rep, 2024).

That dose is far more than most people take. For someone who weighs 70 kg, it works out to about 24.5 g at once. Do not copy it at home.

The bigger takeaway: creatine is not a sleep aid. It is a possible buffer on rough days, and only that.

Can creatine help your mood?

One trial suggests it may support mood alongside treatment. It is not a replacement for care.

Researchers gave 52 women with major depression the antidepressant escitalopram. Half also took 5 g of creatine a day, and half took a placebo. Over 8 weeks, the creatine group improved faster, starting as early as week 2. See Lyoo et al. (Am J Psychiatry, 2012).

Everyone in that trial was under a doctor's care. If you are struggling with low mood, talk to your doctor before you add anything. Never stop or change a prescribed medicine on your own.

Does creatine affect blood sugar?

One small trial says it may, but only with exercise.

Twenty-five adults with type 2 diabetes did a 12-week exercise program. Those who added 5 g of creatine a day saw their HbA1c, a 3-month blood sugar marker, fall about 1.1 points more than the placebo group. See Gualano et al. (Med Sci Sports Exerc, 2011).

That is one trial of 25 people. It has not been widely repeated. If you have diabetes or prediabetes, ask your doctor before trying creatine, since it can change lab results.

Does creatine help bone density?

Not in women, based on the long trials.

The biggest bone trial followed 237 women past menopause for 2 years. They lifted and walked, and some took creatine. Creatine did not change bone density at the hip or spine. See Chilibeck et al. (Med Sci Sports Exerc, 2023).

The 2-year Sales trial above found no bone change either. For bone, calcium, vitamin D, and strength training have far better support. Our post on creatine for menopause covers the bone trials in more depth.

Is creatine good for longevity?

We do not know yet. The only life span data comes from mice.

In a German study of 162 aging mice, the ones fed creatine lived about 9% longer in good health. They also did better on behavior tests. See Bender et al. (Neurobiol Aging, 2008).

Mice are not people. No human trial has tested whether creatine helps anyone live longer. What we do have is evidence that it helps older adults keep muscle when they lift. That matters for staying independent. Read more in creatine for women over 60.

Is creatine safe to take without exercising?

For healthy adults, the trials have found it well tolerated, with or without a workout.

One thing to know before blood work. A 2026 review of 19 trials found creatine raised blood creatinine by about 0.13 mg/dL. eGFR, the estimate of how well your kidneys filter, did not change. See Tsiaras et al. (J Ren Nutr, 2026).

Tell your doctor you take creatine before any lab test. Ask first if you have kidney disease, diabetes, a mood disorder, or take daily medicine. For the full list, see creatine side effects in women.

Who might notice creatine benefits the most?

  • Vegetarians and vegans. They eat little creatine, and the memory trials favored them.
  • Women. The 2024 memory review found a bigger effect in women.
  • Older adults who lift. They get the muscle and strength gains.
  • People short on sleep. Early lab data only.

Pick creatine monohydrate. It is the form used in nearly every study above. Our guide to the best creatine for women shows how to check a label for testing.

Frequently asked questions

Does creatine work if you don't work out?

For the brain, it may. Memory gains in trials showed up without exercise. For muscle and bone, no. A 2-year trial of 200 women taking 3 g a day with no exercise plan found no change in bone, lean mass, or strength.

What does creatine do besides build muscle?

It helps cells recycle quick energy, including in the brain. A 2024 review of 16 trials found a small memory benefit. Early studies point to less mental fatigue and better thinking after sleep loss.

Can creatine help your brain and mood?

For memory, the evidence is moderate and the effect is small. For mood, one 8-week trial found that women with depression on an antidepressant improved faster with 5 g of creatine a day. It is not a replacement for medical care.

Does creatine help with sleep or energy?

It is not a sleep aid. One lab study found a large single dose helped thinking after 21 hours awake. A small trial found 8 g a day for 5 days eased mental fatigue on repeated math tasks.

Is creatine good for longevity?

There is no human evidence yet. Mice fed creatine lived about 9% longer in good health. In people, creatine plus strength training helps older adults keep muscle.
