Creatine on a GLP-1 (Ozempic, Wegovy, Zepbound): how to protect lean muscle while losing weight
Short version: Creatine on a GLP-1 is a reasonable add-on for protecting muscle, but it is not proven yet. No trial has tested creatine in people taking Ozempic, Wegovy, or Zepbound. What we do know: in some GLP-1 studies, 40% to 60% of the weight lost was lean mass, and creatine plus strength training added about 1.37 kg of lean mass across 22 trials in older adults.
- Do first: lift 2 days a week and eat 1.2 to 1.6 g of protein per kg of body weight.
- Then consider: 3 to 5 g of creatine monohydrate a day, with your doctor's OK.
- Watch for: a small jump on the scale from water, and keep drinking fluids.

The scale is finally moving. Good.
But you may have heard the catch. On a GLP-1, some of the weight you lose is muscle, not fat.
So you start reading about creatine. It is cheap, well studied, and everyone at the gym swears by it.
Here is what the research says about taking creatine on a GLP-1, what it does not say yet, and what to do first.
Can you take creatine on a GLP-1?
For most healthy adults, yes. There is no known interaction between creatine and semaglutide (Ozempic, Wegovy) or tirzepatide (Mounjaro, Zepbound).
But "no known interaction" is not the same as "tested." A 2026 paper in Clinical Nutrition put it plainly: "no randomized clinical trial has directly evaluated creatine supplementation during GLP-1-based pharmacological weight loss." See Ostojic and Galyean (Clin Nutr, 2026).
Those authors think creatine is worth testing. They also call their idea "hypothesis-generating," and the lead author advises a creatine maker. So treat creatine as a sensible add-on, not a proven fix.
Ask your prescriber before you start, especially if you have kidney disease.
How much muscle do you lose on a GLP-1?
It depends on the study, and the range is wide.
A 2024 review found that lean mass made up 40% to 60% of the weight lost in some trials, and about 15% or less in others. See Neeland et al. (Diabetes Obes Metab, 2024).
One key detail: "lean mass" is not only muscle. It also counts water, organs, and bone. The same review says MRI studies suggest the muscle loss is mostly in line with the weight lost, and muscle quality may even improve.
Women may lose less. A 2025 joint advisory from four nutrition and obesity groups cites modeling that puts muscle loss at 10% to 15% of weight lost in women, versus 20% to 25% in men, without strength training. See Mozaffarian et al. (Am J Clin Nutr, 2025).
Less is not zero. And women start with less muscle to lose.

Does creatine help preserve muscle on a GLP-1?
It might. The best evidence comes from people losing weight without a GLP-1.
A 2026 trial at Texas A&M gave 64 adults aged 45 to 65, 40 of them women, either 10 g of creatine a day or a placebo for 12 weeks. Some also followed a diet and exercise plan to lose weight. See Chun et al. (JISSN, 2026).
In the diet and exercise group, creatine users gained 1.27 kg of lean mass. The placebo group gained 0.14 kg. Body fat dropped 3.24 points with creatine and 1.87 points without it.
Two cautions. Part of that lean gain is water that creatine pulls into muscle. And the senior author chairs a creatine maker's advisory board.
The bigger picture holds up, though. A meta-analysis of 22 trials in 721 older adults found creatine plus strength training added 1.37 kg more lean mass than training alone. See Chilibeck et al. (Open Access J Sports Med, 2017).
Is creatine and protein a good combination?
Yes. They do different jobs, so they stack.
Protein gives your body the building blocks for muscle. Creatine helps your muscles make quick energy, so you can push a little harder when you lift.
The 2025 joint advisory notes the standard protein target is 0.8 g per kg of body weight a day. During active weight loss, it says higher targets of 1.2 to 1.6 g per kg "have also been proposed."
Here is what that looks like for a 70 kg (154 lb) woman:
- Standard target: 56 g of protein a day
- Weight-loss target: 84 to 112 g of protein a day
- Creatine: 3 to 5 g a day, per the 2021 JISSN review of common creatine questions

Does creatine cause weight gain on a GLP-1?
It can add a little to the scale, and that is mostly water.
A high-dose "loading" week, about 20 g a day, often adds 1 to 3 kg, "mostly attributable to net body water retention," per the 2021 review above. A steady 3 to 5 g a day adds less, more slowly.
That water sits inside your muscle cells. It is not fat, and it is not the puffy kind.

Does creatine help you lose weight?
No, not on its own. Creatine is not a fat burner.
What it may do is help you keep more muscle while you lose fat. In the Texas A&M trial, body fat fell more in the creatine group, but only when paired with exercise and a diet.
A GLP-1 already does the heavy lifting on appetite. Creatine is about what kind of weight you lose, not how much. No supplement can promise a result on the scale.
Are there side effects of combining creatine and a GLP-1?
No combined side effects have been reported, but no trial has looked. Two things are worth knowing.
Your stomach. GLP-1 drugs often cause nausea, in 25% to 44% of people in trials, per the joint advisory. Big single doses of creatine can upset the stomach too. Take 3 to 5 g with a meal, not 10 g at once.
Your kidneys and fluids. The Wegovy label warns about acute kidney injury, mostly in people who got dehydrated from vomiting or diarrhea. In weight-loss trials it happened in 0.4 cases per 100 patient-years on Wegovy, versus 0.2 on placebo.
Creatine does not damage healthy kidneys, per a 2026 meta-analysis of 19 trials. But it raises creatinine, a kidney lab number, by about 0.13 mg/dL. On a GLP-1 your doctor may check kidney labs more often. Tell them you take creatine, so a higher number is read the right way.
And drink fluids. That matters on a GLP-1 with or without creatine.
Should you do high-intensity training on a GLP-1?
Strength training, yes. All-out intensity, ease into it.
The CDC adult guidelines call for at least 2 days of muscle-strengthening activity each week. The 2025 joint advisory names resistance training as a core way to protect muscle and bone on a GLP-1.
The challenge is fuel. You are eating less, so hard sessions can leave you dizzy or wiped out. Start with weights you can lift 10 to 12 times, eat some protein before you train, and build up over a few weeks.
Creatine tends to help most here, when you are lifting. Without training, the benefit is smaller and less certain.
Who should skip creatine or ask first?
- Anyone with kidney disease. The safety data comes from people with healthy kidneys.
- Anyone with frequent vomiting or diarrhea on a GLP-1. Get fluids sorted first.
- Pregnant or breastfeeding women. There is not enough research yet.
- Anyone on several medicines. Let your prescriber or pharmacist check your full list.
