How much fiber do you need on a GLP-1? A daily guide to staying regular

Short version: There is no special fiber number for a GLP-1. Fiber on a GLP-1 should still land near the same target as before: about 22 to 34 grams a day for adults, per the NIDDK. Most women only get about 15. The catch is that you now eat less food, so the fiber has to come from fewer bites.

  • Women 31 to 50: about 25 g a day. Women 51 and up: about 22 g.
  • Pick fiber-dense foods. Raspberries, beans, lentils, and broccoli pack the most fiber per calorie.
  • Go slow and drink more. Add a few grams a week, with extra water every time.

Your appetite shrank. Your plate shrank. And somewhere along the way, your bathroom schedule went quiet.

That's not a coincidence. In pooled trials of semaglutide 2.4 mg, constipation hit 24.2% of people, versus 11.1% on placebo (Wharton et al., 2022). Fiber and water are the everyday levers. But how much fiber, exactly? And how do you fit it into a smaller appetite?

How much fiber do you need on a GLP-1?

The same amount you needed before. No major group has set a GLP-1 fiber target. The 2025 joint nutrition advisory on GLP-1 therapy from four obesity and nutrition groups doesn't give a gram number at all. It says "adequate fluids and fiber from foods should be encouraged."

So use the standard targets. The NIDDK says adults should get 22 to 34 grams of fiber a day, depending on age and sex. The 2020 to 2025 Dietary Guidelines break it down for women:

Those targets come from a simple rule: 14 grams of fiber per 1,000 calories. Here's the trap. On a GLP-1 you may eat far fewer calories than the table assumes. If you shrink your fiber to match, you end up short right when your gut needs help the most. Keep the gram number. Let the calories fall.

Why is it harder to get enough fiber on a GLP-1?

Because you're starting behind and then eating less. Women 20 and older average 15.2 grams of fiber a day, per USDA's What We Eat in America survey. The Dietary Guidelines say more than 90% of women miss their fiber target.

Now add the drug. In one trial, people on semaglutide ate 35% less at a free-choice lunch than people on placebo (Friedrichsen et al., 2021). That's the appetite effect working.

Do the math. If your fiber drops in step with your food, 15 grams becomes about 10. That's less than half of what you need. And it happens while the drug is already slowing your gut down.

The fix isn't eating more. It's making each bite count.

Which foods give you the most fiber per calorie?

When your appetite is small, fiber per calorie matters more than fiber per serving. We took the numbers from the Dietary Guidelines' food sources of dietary fiber table and worked out the grams per 100 calories.

Berries and green vegetables win on a small appetite. Beans and lentils give you fiber and protein in the same bite, which helps on a GLP-1 too. Prunes still earn a spot. The joint advisory names them as a helpful food for constipation, even though they're lower on this list.

How do you add fiber without feeling worse?

Slowly. The NIDDK's constipation diet advice says to "add fiber to your diet a little at a time so your body gets used to the change." It also says to drink plenty of liquids to help the fiber work better.

Timing matters on a GLP-1, too. The joint advisory notes that skipping fatty or high-fiber foods in the first few days of treatment can ease nausea. So if you just started or just raised your dose, wait until your stomach settles. Then build up.

Here's one way to do it. It's a sample plan, not a prescription. Adjust it to how you feel.

Add a glass of water each time you add fiber. If gas or cramping shows up, stay at that week's level for a few more days before you move on.

Can you take a fiber supplement with a GLP-1?

Yes, for most people. The joint advisory says fiber supplements or capsules "may also be beneficial." A clinical review on GLP-1 gut side effects tells patients to increase water, fiber, and movement (Gorgojo-Martínez et al., 2022).

The evidence for fiber supplements is decent. In a review of 16 randomized trials, 66% of people with constipation responded to fiber, compared with 41% in the control groups. Psyllium at more than 10 grams a day, taken for at least 4 weeks, looked best (van der Schoot et al., 2022). Fiber also caused more gas than placebo, which is another reason to start low.

Food should still do most of the work. A supplement fills the gap on days your appetite won't cooperate. If you take other medicines by mouth, ask your pharmacist how to space them from a fiber supplement.

What is the easiest way to track fiber on a GLP-1?

Use the label you already have. The FDA's Daily Value for fiber is 28 grams. So every 10% DV on a Nutrition Facts label is about 3 grams.

  • Aim for about 90% DV a day. That's 25 grams, the target for women 31 to 50. If you're 51 or older, about 80% gets you to 22.
  • Count anchors, not every crumb. Pick three fiber anchors a day, like berries at breakfast, beans at lunch, and a vegetable at dinner. Three good anchors can cover 20 grams.
  • Log for one week, then stop. An app helps you learn your numbers. After a week, most people can eyeball it.

This works the same on semaglutide (Ozempic, Wegovy), tirzepatide (Mounjaro, Zepbound), or any other GLP-1. Your gram target doesn't change with the brand.

How do you stay regular on a GLP-1?

Fiber, fluids, and movement, every day. The GLP-1 side-effects review lists all three. But "regular" doesn't have to mean once a day. The NIDDK defines constipation as fewer than three bowel movements a week. Soft, easy stools a few times a week can be normal for you.

Constipation on these drugs can also last. In the same review, it lasted a median of 47 days in people with obesity. On tirzepatide, the Zepbound label lists constipation in 11% to 17% of people across doses, versus 5% on placebo. So build the habit early. Don't wait until you're backed up.
