Why medicine is not one-size-fits-all for men and women

For decades, the "standard" medical patient was a 150-pound male. From clinical drug trials to the way we diagnose heart attacks, the female body was often treated as a "smaller version" of a man’s. But as any woman knows, our biology—from our fluctuating hormones to our cellular metabolism—is vastly different.

Why medicine is not one-size-fits-all for men and women

Short version: The sex differences in medicine are not about size. They sit in your stomach, your liver, your kidneys, and your immune system. Same pill, different body, different exposure.

  • Over 100 commonly prescribed drugs behave differently in women and men. Only a handful carry different dosing advice.
  • Gastric and colonic emptying is slower in women, fat mass is higher, and kidney filtration is lower. All three change how long a drug stays.
  • In 427 human livers, 77 drug-handling genes differed by sex. CYP3A4 ran 1.73-fold higher in women.
  • In 1,143,513 heart attack patients, 42.0% of women arrived without chest pain, against 30.7% of men. In-hospital death was 14.6% versus 10.3%.

You take the standard dose. You feel wrecked for a day and a half.

Someone tells you that you are sensitive. Maybe you start believing it.

Here is the more accurate word. Exposure. The sex differences in medicine are mostly a question of how much drug ends up in your blood and for how long, and your body answers that question differently. Over 100 commonly prescribed drugs behave differently in women and men. Almost none of them say so on the label.

What are the sex differences in medicine that actually matter?

Four systems, and they compound.

Absorption. Distribution. Metabolism. Clearance. Every drug you swallow passes through all four, and each one runs differently on average in a woman's body.

Sommer and colleagues lay it out in World Psychiatry. "Gastric and colonic emptying is slower," they write, which gives a pill more time to be absorbed.

Women "on average, have more fat tissue, which can lead to stacking of lipophilic pharmaceuticals," per the same paper. Fat-soluble drugs park there and leave slowly.

And the exit is narrower. "Renal blood flow, glomerular filtration, tubular secretion and resorption are all lower in women."

More time in. More places to sit. Slower way out.

What do the liver enzymes actually do?

They break drugs down. And they are not set to the same levels in every body.

Yang and colleagues measured this directly in Journal of Drug Metabolism and Toxicology. They looked at 427 human livers: 234 male, 193 female.

77 drug-metabolizing and transporter genes differed significantly by sex.

CYP3A4 alone handles a large share of prescription drugs. "CYP3A4 appeared to have a higher activity in women while CYP1A2 and CYP2E1 have higher activities in men," the authors write.

Estrogen tunes several of these. So the same woman can clear the same drug differently before and after menopause.

Can one drug show the whole problem?

Olanzapine does. It is an antipsychotic, cleared largely by CYP1A2.

Estrogen slows CYP1A2 down. So the drug leaves more slowly.

The result, per Sommer and colleagues: "Blood levels may be about two-fold higher in pre-menopausal women than in men with equal dosing."

Two-fold. Same prescription, twice the blood level, no warning.

The famous version of this story is a sleep drug, and the FDA did eventually halve the recommended dose for women. We tell that one in full in why women get the wrong drug doses.

If the standard dose flattens you, that is not a personality trait. It is a clearance rate.

Does a heart attack look different in women?

Often, and the gap is measured in deaths.

Canto and colleagues studied 1,143,513 heart attack patients in JAMA: 481,581 women and 661,932 men.

42.0% of the women arrived without chest pain. For men it was 30.7%.

In-hospital death was 14.6% for women and 10.3% for men.

The gap was widest in the youngest patients. Adjusted odds of presenting without chest pain ran 1.30 under age 45, then narrowed to 1.03 at 75 and over.

So the textbook symptom is a male-typical symptom. A woman having a heart attack may feel shortness of breath, nausea, or pain in her back or jaw instead.

Learn those signs. Call emergency services anyway if something is badly wrong.

Is the immune system different too?

Substantially. This is the clearest sex difference in all of medicine, and it cuts both ways.

"Females generally exhibit stronger innate and adaptive immune responses than males, enhancing pathogen defense but increasing susceptibility to autoimmunity," write Kim and colleagues in the International Journal of Molecular Sciences.

The wiring is partly chromosomal. The X chromosome carries about 11 times more immune-related genes than the Y, and 20% to 30% of X-linked genes escape the switching-off process that normally silences one copy.

You can see the cost in the disease counts. About 80% of people diagnosed with an autoimmune disease are women, per a review by Angum and colleagues in Cureus.

Sjögren's syndrome runs 9 to 1. Lupus 7 to 1. Rheumatoid arthritis 3 to 1.

A stronger defense force is also a force with more chance of turning inward.

How many drugs have sex-specific dosing?

Almost none. That is the whole gap in one sentence.

"Only a handful of drugs...have different dosing recommendations for women, while there are over 100 commonly prescribed drugs with unequal pharmacokinetics between men and women," Sommer and colleagues write.

Their conclusion is blunt. That mismatch "suggests that women are at high risk for both over- and under-dosing."

Note the second half. Under-dosing is also a real outcome, and it looks like a drug that simply did not work.

What can you do at your next appointment?

Ask questions that force the sex-specific data into the room. Five that work:

  • Was this drug studied in women, and were the results reported separately by sex?
  • Is this drug fat-soluble? If so, how long does it stay in my system?
  • I am currently menstruating, on hormonal birth control, or postmenopausal. Does that change my dose?
  • Can we start at the low end and titrate up, rather than starting at the standard dose?
  • Which side effects do women report more often with this drug?

Write down what happens at the dose you are given. Timing, severity, how long it lasts.

That record is data. A dismissed complaint is easy. A two-week log is harder to wave off.

What should I ask my doctor about dosing?

Ask five things. Was the drug studied in women, with results split by sex? Is it fat-soluble, and how long does it stay? Does your current hormone status change the dose? Can you start low and work up? Which side effects do women report more often?

Then keep a written log. Timing, severity, how long it lasts, at the dose you were given.

These questions matter for a structural reason. Over 100 common drugs act differently in the two sexes. One 2024 analysis says that puts women "at high risk for both over- and under-dosing." Under-dosing is easy to miss. It looks like a drug that just did not work.
